If you would like to report a concern, please fill out the contact form below.

We are happy for individuals to report information anonymously, but in doing so it severely limits the action that we can take, so please provide details that will allow us to discuss your concern with you more fully. By submitting information and providing your details you agree to us contacting you to discuss your concern.

All contact is confidential within best practice guidance and your personal data will be kept securely in line with the UK General Data Protection Regulations and the Data Protection Act 2018.
